Guidelines for Securing Insurance Reimbursement:

Step 1: Contact your insurance provider

You can find the member services phone number on the reverse side of your insurance card..

Step 2: Determine if you have out-of-network benefits

Ask the representative: "Does my plan provide out-of-network benefits for mental health care, especially for outpatient therapy at a psychologist's office?"

Step 3: Confirm if a deductible applies before coverage begins

Ask the representative: "Is there a deductible for out-of-network mental health services? If there is, how much do I need to pay before my health plan starts reimbursing my out-of-pocket costs?"

Step 4: Find out your plan's reimbursement amount

Ask the representative: "What is the maximum amount my plan will reimburse for mental health service code 90837 with a psychologist?" If the response isn’t clear, follow up with: "What is the maximum allowed amount for code 90837, and what percentage of that will my plan cover?" (This percentage represents your reimbursement.)
